OTS is complex because there are both physiological abnormalities and often psychological challenges stemming from the stress of competition, family and social relationships, and other life demands. There are no set diagnostic criteria for OTS, because symptoms vary from athlete to athlete and are highly individualized. Athletes with suspected OTS should seek medical attention to rule out other confounding conditions and to develop a sound plan for recovery.

Although the body makes and uses small amounts of ketone bodies at all times, ketogenesis can be ramped up to meet ATP demand when glucose is low and normal fatty acid oxidation is compromised. Excessive acetyl-CoA molecules (from fat metabolism) are used to produce the ketones acetoacetate and beta-hydroxybutyrate in the liver (figure 2.15). These acidic and water-soluble ketones are easily transported in the bloodstream to nonhepatic tissues for utilization as fuel, an energy source for the brain, intestinal mucosa, heart, skeletal muscle, and kidneys.

Vitamins are necessary for metabolism, proper growth and development, vision, and organ and immune functioning. They also aid human performance by facilitating energy production; supporting muscle contraction and relaxation, as well as oxygen transport; building and maintaining bone and cartilage; building and repairing muscle tissue; and protecting the body’s cells from damage. The human body can make vitamin D, and gut bacteria can make vitamin K; all other vitamins must be consumed in the diet to meet dietary requirements. Although deficiencies or insufficiencies in certain nutrients can impair training adaptations and performance, no current data indicate that excess nutrient intake, beyond the requirements for general health, will improve athletic performance (1). In some cases, excess consumption of specific vitamins may interfere with performance, training adaptations, and recovery. Figure 6.1 illustrates the importance of many vitamins discussed in this chapter.
